Referrals

  • Generally 65+ year old patients having complex medical issues; we are happy to see younger people if they are experiencing geriatric syndromes

  • After the referral is made, we will contact the patient to schedule an appointment. The patient does not have to do anything else. They can feel free to contact us if they have not heard from us in 2 business days.

  • I will send your office a letter with my note electronically.

    Additionally, we can communicate however often you’d like (whether it is when you have a specific question or every time I see a mutual patient). We will make sure to remember your preference.

  • I provide solely consulting services.

    I will not be serving as the PCP for any patients I see.

  • I do not bill any Medicare Welcome or Annual Wellness Visits.

    Let us know if there are any other codes that have billing limits that you want to make sure we don’t do, we’re happy to work with you.

Records Requests

  • Call, email, or fax me a request with this information:

    • Type of records requested

    • Date for which records are being requested

    • Reason for request (legal, medical management, etc.)

  • Please send all records to our HIPAA-compliant email or fax.
